Lifestyle and Wellness
The Lifestyle Care Manager will conduct a HomeLonger Lifestyle and Wellness Program assessment to provide a written plan and recommendations for necessary services that will help optimize and prolong well-being and independence. All HomeLonger Lifestyle Care Managers are required to obtain certification from the National Academy of Certified Care Managers and maintain membership in the National Association of Geriatric Care Managers.
