Membership Levels

Many lifestyle services today are offered as a fragmented, often frustrating network of providers. By bundling services that most adults feel are important, HomeLonger has taken the frustration of finding reliable providers out of the equation, and offer comprehensive, trustworthy service providers that make a difference.

HomeLonger offers three levels of care and wellness support. The common element is the HomeLonger Lifestyle and Wellness Program. This program is developed in partnership with the client and Lifestyle Care Manager. The frequency and intensity of care services increases as the need for such services changes. Membership packages can be customized and changed as needs change.

Samples of HomeLonger Membership Levels are:

LEVEL 1: INSPIRED LIVING

This level of membership is ideal for the person or family who does not currently have a lot of specific needs but wishes to plan ahead and have access to HomeLonger services when necessary.

Membership may include:

HomeLonger Lifestyle & Wellness Program
HomeLonger Concierge
Lifestyle Care Manager in-home assessment
HomeLonger Wellness Organizer
Up to 60 hours annually for personal support services
HomeLonger monthly wellness events
HomeLonger monthly newsletter
Customized reminder care calls
Access to HomeLonger preferred provider network for premier service provider discounts
Access to ala carte services

LEVEL 2: INSPIRED LIVING HEALTH

This membership level is for someone who has a few health problems and could use light support services to make their life easier while providing peace of mind.
Membership may include all benefits of Inspired Living Level 1, plus:

Lifestyle Care Manager visits
Up to 120 hours annually for personal support services
Monthly wellness call
Monthly family wellness updates
In-home fall risk/safety analysis
Medication management program

LEVEL 3: INSPIRED LIVING CARE

This level of membership is for someone who could use help in a number of areas to maintain their desired level of independence in the home. It is also perfect for a personal that is recovering from an illness or has had a recent hospitalization.
Membership includes all benefits of Levels 1 and 2, plus:

Significant additional personal support services hours (customized per client)
Care coordination services by a licensed health professional
Monthly wellness visits
Weekly wellness calls
Annual vascular and bone disease screenings
Annual in-home nutritional analysis & meal planning
Access to home visits by licensed medical providers such as a nurse practitioners
